Some children go years without a single cavity. Others, despite what looks like reasonable brushing habits, develop new cavities at nearly every visit. The difference usually isn’t effort — it’s risk factors that most parents were never told to look for. That’s the idea behind caries (cavity) risk management: instead of giving every family the same generic advice, we assess your specific child’s risk factors and build prevention around them.

What goes into a caries risk assessment:

  • Diet patterns, especially frequency of sugar and carbohydrate exposure (grazing on snacks throughout the day is often worse than one dessert, because it keeps the mouth acidic longer)
  • Fluoride exposure, from water source, toothpaste, and any supplemental treatments
  • Saliva flow and composition, which affects how well the mouth naturally neutralizes acid and remineralizes enamel
  • Bacterial load, since cavity-causing bacteria can be transmitted between family members through shared utensils or cups
  • Enamel quality, including developmental defects that make some children’s teeth inherently more vulnerable
  • Home hygiene routine, including whether a parent is still assisting with brushing (recommended until around age 7 or 8)

Prevention tools we use based on risk level:

  • Professional fluoride varnish applications at routine visits
  • Dental sealants on molars, which physically block the deep grooves where cavities most often start
  • Prescription-strength fluoride toothpaste for higher-risk kids
  • Xylitol products, which can reduce cavity-causing bacteria
  • Dietary counseling focused on realistic changes, not just “eat less sugar”
  • More frequent recall visits (every 3-4 months instead of 6) for high-risk children

The goal of risk-based prevention isn’t to make parents feel like they’re failing when a cavity shows up. Some children are simply working with harder biology than others, and the right response is a more tailored plan, not more guilt.
